    7 week session begins January 7th through February 25th (No class 2/18)
    DAY: Tuesday
    TIME: 6:00 - 6:50 PM
    AGES: 4-7

    ( All students should wear shin guards and bring their own ball.
    3-4 year olds use size 3 ball and 5-7 year olds use size 4 ball.)

    Sports Stars Skills & Drills Policies and Waiver-


    ​Weather
    Due to the unpredictable Buffalo winter conditions, clinics and training sessions will not be cancelled unless the coach or Sports Stars Skills & Drills LLC. finds it unsafe for the players.  If the Kenton School district is closed due to weather, all sessions would be cancelled as well.  We will attempt to reschedule if possible, but due to scheduling with gym time, we normally cannot reschedule.  Please note, no refunds will be given for weather related cancellations.  

    Refunds
    Please be aware that after a session starts there will be no refunds.  If a player needs to withdraw before the start of a session, they will receive a full refund minus a $15.00 registration fee.   Sports Stars Skills & Drills LLC reserves the right to cancel any session due to insufficient enrollment.  Upon cancellation, a full refund would be made.

    Waiver and Release of All Claims and Assumption of Risk
    I recognize and acknowledge that there are certain risks of physical injury to participants in this clinic, and I agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor, child or I may sustain as a result of said participation. I further agree to waive and relinquish all claims I or my minor child may have (or accrue to me or my child) as a result of participating in this clinic against Sports Stars Skills & Drills LLC. including its officials, volunteers, and employees.  I do hereby fully release and forever discharge Sports Stars Skills & Drills LLC from any and all claims for injuries, damages or loss that my minor child or I may have or which may accrue to me or my minor child and arising out of, connected with, or in any way associated with this clinic.

    Warning of Risk:
    Recreational programs and activities are intended to challenge and engage the physical, mental, and emotional resources of each participant. Despite careful and proper preparation, instruction, medical advice, conditioning, and equipment, there is still a risk of serious injury when participating in any recreational program. Understandably, not all hazards and dangers can be foreseen. Depending on the particular activity, participants must understand that certain risks, dangers, and injuries due to inclement weather, slipping, falling, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defect, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to indoor, outdoor, and water related recreational programs exist. In this regard, it must be recognized that it is impossible for Sports Stars Skills & Drills LLC. to guarantee absolute safety.

    Photo/Video Authorization:
    I hereby give my consent for Sports Stars Skills & Drills LLC. to use photos/video coverage of myself and/or minor child in future Sports Stars Skills & Drills program guides, flyers, videos, websites, etc.  I understand that Sports Stars Skills & Drills LLC. staff may take photos/video coverage during its programs and events, and their participants from time to time and that these photos/video coverage remain the property of Sports Stars Skills & Drills LLC.  
     
    Return to Play Waiver for Infectious Disease - COVID-19
    ​    ​IN CONSIDERATION of being permitted to participate in any way, I, for myself for personal representatives, assigns, heirs, and next of kin acknowledges and agree that:
    ​1. The risk of injury and/or illness from the activities involved in the program is significant, including potential for permanent paralysis or death, and while particular rules, equipment, and personal discipline may reduce the risk, the risk of serious injury does exist. 
    ​2. The risk to have contact with individuals, wo have been exposed to and/or have been diagnosed with one or more communicable diseases, including but not limited to COVID-19 or other medical conditions, diseases, or maladies does exist, and it is impossible to eliminate the risk that I could be exposed to and/or become infected through contact with or close proximity with an individual with a communicable disease. 
    ​3.  I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume all full responsibility for my participation. 
    ​4. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence and participation, I will remove myself from participation and bring such to the attention of the nearest coach immediately. 
    ​5. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS SPORTS STARS SKILLS AND DRILLS LLC, their facilities, owner, managers, coaches and employees, WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH or loss of damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASES OR OTHERWISE. 

    ​I HAVE READ THIS RELESASE OF LIABILITY AND ASSUMPPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY AGREEING TO IT ON MY OWN BEHALF OR FOR MY SON OR DAUGHTER AS LEGAL GUARDIAN TO PARTICIPATE IN ANY SPORTS STARS SKILLS AND DRILLS LLC ACTIVITY, AND I SIGN FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

    By selecting this, I am indicating that I have read the above document, I understand it completely, and I agree to be bound by its terms.
